You need to fill in the KOPENAS membership form and the ANGKASA form and also include a copy of the identity card, two months' copy of the latest salary slip certified by the employer and a registration fee of RM20.00 (RM10.00 registration fee and RM10.00 shares) (Please contact the head office KOPENAS to get the ANGKASA form)
